By checking Terms and Conditions box, I (self, player, parent and/or guardian named on this registration) read, understand, and assume all responsibilities as outlined in Sports Center Camp and Policies (as seen below)

LIABILITY WAIVER

In consideration of being allowed to participate in any way in the Sport Center Camp LLC skating clinic and/or camp, I release and discharge Sport Center Camp LLC, the owner and any other owner, employee, agent, volunteer or coach participating in the clinic, camp and related events and activities, from any and all liability, claims or demands. I acknowledge, appreciate and agree with the following: 1) The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury or death does exist; 2) I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the Releasees or others, and assume full responsibility for my participation; 3) I willingly agree to comply with the stated and customary terms for participation in the camp or clinic, however, if I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; 4) I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release, indemnify, and hold harmless the Sport Center Camp LLC, its members, owners, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners of the premises used to conduct the clinic/camp (“Releasees”), with respect to any and all injury, disability, death, or loss or damage to person or property, whether arising from the negligence of the Releasees or otherwise, to the fullest extent permitted by law; 5) I hereby agree to submit to binding arbitration any and all claims which I believe I may have against Sport Center Camp LLC, the Releasees or the facility arising from my activities at the facility. The arbitration shall be pursuant to the rules of the American Arbitration Association. The arbitrators shall apply the Federal Rules of Evidence to all proceedings. Arbitration shall be commenced within one (1) year from the date on which any alleged claim first arose.

Further, the arbitration shall be held in Denver, Colorado, unless otherwise mutually agreed to by all the parties. The submission to the American Arbitration Association shall be unlimited and the arbitration award may be enforced by any court of competent jurisdiction. I have read this release and waiver of liability, I assume all risks associated with participating in this camp/clinic, I fully understand its terms, I understand that I am giving up substantial rights by signing it, and I sign it freely and voluntarily without any inducement.

PRIVACY POLICY

Data Collection and Purpose Specification

The Online Registration feature is intended for use by those wish to participate in a Besa Hockey and Sport Center Camp LLC program. The information collected during the registration process will only be used for internal purposes. The information collected will not be displayed publicly. We use information collected only to fulfill your requests for services and communicate additional relevant program and company information.

Financial & Credit Card Information

For your convenience, to facilitate player registration, Sport Center Camp LLC uses a 3rd Party financial institution (Paypal) to process your credit card information. This process is completed through a direct and secure link to Paypal. You do not need a Paypal account to use this service. Please note that such information is NOT captured by Sport Center Camp LLC or our web provider during any part of the transaction process.

REFUND POLICY

Sport Center Camp LLC Event, Camp, Clinic, Lesson, & Speed Testing Registration Fees are NOT refundable.

An exception to the no-refund policy is the proration of unused fees for an event-ending injury (medical refund)

Executive Staff will review requests for medical refunds. All refund requests must be made in writing and have physician documentation supporting suspension or termination of training and game play.

In all cases, a $100 administration fee will be charged for any refund given.

No refund requests will be considered within 5 days of the start date of an event for any reason, except for medical refunds.

The Executive Staff may also review requests for refunds for other extraordinary circumstances at their discretion.

PHOTO AND LIKENESS WAIVER

I hereby grant the Sport Center Camp LLC permission to use my likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration.

I understand and agree that all photos will become the property of Sport Center Camp LLC and will not be returned.

I hereby irrevocably authorize the Sport Center Camp LLC to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.

I hereby hold harmless, release, and forever discharge Sport Center Camp LLC from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
